Renewal for 2010

    I am a little bit out the the loop since I have spent the summer in Seattle and getting set up in my new group practice in Phoenix so please forgive me for a dumb question. For a number of reasons, I went to one credit card (American Express) and cash/checks, so can you send me a bill or where do I write and send the check. Thanks in advance. Bob

    There was an e-mail sent out for renewal as well as a letter in the mail. The take credit (only visa and mastercard) and checks or money orders.

    Just cross out the 2009 on Lee's form and write in 2010~!

    NOTE: For those of you who have faxed in your form, and have NOT gotten an email verification of same, please call JRT, 1-800-998-1110. I faxed in my form 11/13/09, and double checked, their fax machine was DOWN one day in November, they never received my fax. I re-faxed today, and got confirmation. Please renew by DECEMBER 31, 2009~!
