Media gallery, upload pics

I uploaded ONE picture "DAVE.JPG".

The result was 2 (!) pictures with the same name "DAVE4.JPG".

??? problem ???

Have had that a few times as well. One time I even deleted the double and the picture was gone, but the residue of the photo was still there.
