When the thermostat was installed, so you think any of the gasket got lodged in the housing?

If the valve isn't opening in the thermostat...it would run hot.

Burped the cooling system?

If your fan is working correctly then I'm guessing that the
thermostat has gone bad and is not opening all the way

As what was said above, it may be causing the hose to blow off.

Make sure that the overflow hose is not clogged. If it is, incresed pressure in the system will not be relieved by the overflow tank.
