Door hits hood?

I have seen several cars with this damage.

Is there something to avoid doing to keep this from happening, like opening the door with the hood partially open?

That would be pretty hard to get out of your car without opening the door to pop the hood!

It seems like the hinge on the door could be bent....possibly from leaning on the door when you get in or out?
